G-325A Question

I downloaded the latest G-325A form (expires 06/30/2011)

I am trying to put a date of birth (mm/dd/yyyy) in the father and mother section of the form.
It allows me to put in a date of birth but a little black square with a white + sign appears in the bottom right corner. This square obscures the last two digits of the year.
When I print out the form the black square appears on the form and it is not possible to see the year.
The black square appears on all other areas of the form where it asks for a date of birth.

Has anyone else had this problem?

Should I just write in the date of birth in pen?

Yes

btw, determine the latest version of the form (any form) by the revision date in the bottom right corner, not by the OMB approval expiration date in the upper right corner. Expiration of OMB approval of the form does not mean that the form has expired and can no longer be used.
